Cardiovascular-kidney-metabolic (CKM) syndrome is a health disorder made up of heart disease, kidney disease, diabetes and obesity, leading to poor health outcomes. Groundbreaking new therapies exist, but many patients experience fragmented care. The ÌÇÐÄVlog is committed to improving outcomes for CKM patients, placing each person’s needs at the center of treatment.

About the Initiative

In 2024, the ÌÇÐÄVlog launched the CKM Health Initiative, answering the call to action laid out in a 2023 presidential advisory defining CKM Syndrome. The 4-year initiative provides a path forward for patients, communities and health care professionals, with a focus on care coordination and equipping patients for their health journey.

Why CKM Health Matters

Cardiovascular, kidney, and metabolic conditions often occur together—yet are too often treated apart. This fragmented approach leads to missed diagnoses, delayed treatment, and worsening outcomes. In its later stages, CKM Syndrome can lead to heart failure, kidney failure, stroke, and a loss of life.

By recognizing CKM as a connected syndrome, we can shift from reactive care to earlier detection, coordinated treatment, and better outcomes for the more than 89 million at risk in the U.S. alone.

The CKM Initiative is launching new education for professionals, created with experts across specialties and built to support cardiologists, nephrologists, endocrinologists, and primary care providers in delivering more coordinated care.

Driving Clinical Implementation

We’re building the infrastructure to turn science into practice—offering implementation tools for care teams, developing CKM center & coordinator certification standards to recognize CKM-aligned care, and working with 150 select sites to shape and refine real-world guidance.
